2025 Community Health Needs Report Now Available

In August and September 2025, more than 5,000 Community Health Needs Assessment surveys were mailed and five focus groups conducted to accurately identify community needs specific to our unique demographics and region. Mailed surveys went to a random sampling of older adults with emphasis on reaching an equal percentage of adults age 60 and greater within each county. Individual and community needs vary due to factors including proximity to larger cities and health care, insurance coverage, eligibility for federal and state assistance, personal health and mobility, access to transportation and healthy meals, and more. Using this information to develop a comprehensive four-year plan, the Agency seeks to strengthen existing partnerships and develop new ones toward providing resources and addressing gaps to help residents throughout our region to age in safety and contentment in their preferred homes.

The top concerns revealed in this 2025 Needs Assessment Survey revolve around:

• Finances – affordability of medical bills, medications, utilities, meals, home-care services, and housing;
• Urban vs Rural resources – access to health care, transportation, meals, more;
• Social Connection – companionship vs isolation; and
• Awareness of Resources – knowing who to call for help in an emergency and pre-planning, and distributing authoritative information.

The Agency’s 2027-2030 Strategic Area Plan for the Ohio Department on Aging will utilize existing resources, services, community and organizational partnerships, as well as identify new outlets, to:

• address needs through existing programs, partnerships, and funding;
• identify gaps, available funding, and best partnerships to fill the gaps;
• develop new ideas and methods of outreach and connection to minimize isolation and maximize awareness of resources.

The Agency appreciates the time and input from residents throughout our region for taking the time to complete the extensive survey and participate in focus groups to help us gauge where our programs and resources are most effective, and where we need to focus to alleviate gaps in care.
