Public Notice: Heightened Scrutiny for Prospective Assisted Living Waiver Providers

The Ohio Department of Aging (ODA) is accepting comments on three assisted living settings being submitted to the Centers for Medicaid and Medicare Services for heightened scrutiny review.

• Monarch Meadows Nursing and Rehabilitation, LLC, Seaman, OH;
• The Lodge at New Dawn, Canton, OH; and
• Villa Vista Royale, Steubenville, OH.

The settings are new providers seeking ODA-certification to furnish the Medicaid assisted living service.
