Comprehensive Assessor Job Application

We are seeking an assessor to conduct multifaceted, comprehensive assessments of consumers to identify unmet needs, link to long-term care options, and determine eligibility for a variety of service alternatives, including but not limited to PASSPORT Home Care enrollment.

Must have good assessment skills and be a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed Social Worker (LSW) with a minimum of one year prior experience in-home care for elderly and/or disabled adults, home health care, medical social work, geriatrics/gerontology, or other substitutes as approved by the ODA. Possess knowledge and skills necessary to complete assessments and activities consistent with PASSPORT standards and procedures.
