The Area Agency on Aging is seeking a dedicated Comprehensive Assessor to support individuals and caregivers in accessing long-term care services and community-based resources. This role is responsible for conducting in-depth, multi-dimensional assessments to evaluate physical, functional, social, and environmental needs, determine eligibility for Medicaid and waiver programs, and develop person-centered care plans. This position requires strong clinical judgment, communication skills, and the ability to manage documentation and compliance requirements in accordance with state and agency guidelines.
Must have good assessment skills and be a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed Social Worker (LSW) with a minimum of one year of prior experience in-home care for elderly and/or disabled adults, home health care, medical social work, geriatrics/gerontology, or other substitutes as approved by the ODA. Possess knowledge and skills necessary to complete assessments and activities consistent with PASSPORT standards and procedures.
