We are seeking support staff to assist case managers with seniors needing long-term care services to remain in the community. Functions in coordination and support of the Case Management staff. Activities include client and community agency contacts to ensure the approval and ongoing eligibility for Medicaid.
Associate Degree is desired, additional equivalent experience may be substituted for part of the education requirement. Excellent verbal and written communication skills with persuasive ability. Must reflect customer focused care approach in all communication. Ability to prioritize tasks. Possess clinical knowledge and interview skills necessary to complete physical, functional, social, environmental, financial and mental health assessment. Computer literacy, including but not limited to Microsoft Suite. Must be knowledgeable of universal precautions and infection control. Knowledge of basic math. Awareness of community resources and services.
