Care Transition Coach

Provide coaching methods and models to members with acute care needs, chronic illnesses or any other managed care identified health issues to reduce the likelihood of readmission, improve health outcomes, and lower health care costs. Coach assists members with new, costly and or changing health-care needs transitioning from hospital settings to home settings for the entire 9 county region

Licensed Social Worker or Registered Nurse holding active license in the state of Ohio or other substitute as approved by the AAA Executive Director. OR Contractual requirements by program.
