CASE MANAGER

The Area Agency on Aging team is seeking caring, compassionate Licensed Social Workers and Registered Nurses Case Managers to help to assist older adults needing long-term care services to remain in the community. Must have good assessment skills and be a Registered Nurse (RN) or Licensed Social Worker (LSW) holding an active license in the State of Ohio.
