Seeking Proposals for Transportation in Shelby County

The Area Agency seeks proposals from service providers who are interested in receiving a four-year contract for fiscal year January 1, 2019 – December 31, 2022 through a competitive Request For Proposal process for provision of Medical Transportation for older adults, age 60 and older, residing in Shelby County. The contract will be on a unit reimbursement basis of $1.75 per mile and the planned annual award amount is $10,000. The planned funding for this service is through Older Americans Act (Title III) federal grant award funding. This annual award requires the contracted provider to match by a 15% cash or in-kind match per service contract awarded.

The proposal (submitted as both a hard copy and electronic version) is due by no later than September 10, 2018 at 5:00 pm.
