RN or LSW

We are seeking RNs and LSWs to assist seniors needing long-term care services to remain in the community. Must have good assessment skills.

 

  • Full-time
  • Monday through Friday
  • No weekends, evenings, or holidays

Comprehensive Assessor

RN or LSW to conduct multifaceted comprehensive assessments of consumers to identify unmet needs, link to long-term care options, and determine eligibility for a variety of service alternatives, including but not limited to PASSPORT Home Care enrollment.

RN or LSW with a minimum of one-year prior experience in-home care for elderly and/or disabled adults, home health care, medical social work, geriatrics/gerontology, or other substitutes as approved by the ODA. Possess knowledge and skills necessary to complete assessments and activities consistent with PASSPORT standards and procedures.

Clinical Support Specialist

We are seeking support staff to assist case managers with seniors needing long-term care services to remain in the community.
Functions in coordination and support of the Case Management staff. Activities include client and community agency contacts to ensure the approval and ongoing eligibility for Medicaid.
Prior experience in social services is beneficial. Associate Degree desired in Gerontology or related field. Computer literacy a must. Excellent communication Skills. Ability to prioritize tasks.
